Daily Phone Call (Service Option 6)
Day and Night On-Call Service
This service is often utilized by clients who don’t need Care Management or On-Going Consultation services because family is nearby. But when family members travel out of town, this short-term service brings peace of mind. This service can be combined with the Daily Phone Call Service described below. With this service, the Registered Nurse is on-call for the client and/or client’s caregiver day and night for a defined period. When a change of condition occurs, the client or caregiver calls the nurse who responds by visiting to assess the situation. The nurse outlines care options, communicates with family, and takes appropriate action. The nurse stays with the client until the client is stable and then, with family permission, may arrange for additional helpers to stay with the client.

Daily Phone Call Service
This service is a pre-arranged “hello, how are you” call. It’s especially useful for people who are isolated from others by distance, disability, or weather. The Registered Nurse initiates the call at an agreed upon time. A pre-arranged response protocol directs the nurse, should the client not respond to the telephone call. This service can be arranged for short or long term and can be combined with other services such as Day and Night On-Call Service, described above.
